Model Release Forms
A word about Model Release.
Firstly let's remember that most models will charge you an hour for them to sign so we want to be able to use their pictures to earn our income. You may find several years down the line that a photo set you took that wasn't at the time saleable becomes so and you want to be able to use those pictures. Model release does not need to be complicated and the release one to three are pretty good if very wordy. It all depends how tight you want to tie things up. An example would be.
You hire a young unknown model and pay for a release which she signs. She then goes on to become a very famous actress who doesn't want anyone to know about her nude modelling past. Those pictures you have suddenly become very saleable and valuable as the tabloid press will probably pay handsomely. Of course manners would ask that you offer the actress the pictures first and let her know what is going on. So you can see how important model release is. Also you will have not a snowball in hells chance of selling work without one.
Finally on this business of age and proof of age. Obviously this is only really necessary when the model could appear to be under the age of consent. If she clearly looks over age then I am of full age and have the right to contract in my own name. will suffice nicely. Remember though if it is even vaguely doubtful request age of birth and proof.

In consideration of compensation as indicated below, receipt of which is hereby acknowledged, I Suzy Model do hereby irrevocably authorise, Joe Photographer his legal representatives, assigns, and those acting under his permission and on his authority, to copyright, publish, and use in all forms and media and manners for advertising, trade, promotion, exhibition, or any other lawful purpose whatsoever, still, single, multiple or moving photographic images or pictures, video recordings, audio recordings of me in which I may be included in whole or in part, alone or in conjunction with other persons, or composite or distorted in character, or form, or based on or involving any stunt poses, in conjunction with my own or a fictitious name, or reproductions thereof in colour otherwise, or in derivative works made through any medium.
I do hereby waive any right that I may have to inspect or approve the finished product or advertising or other copy that may be used in connection therewith or the use to which they may be applied.
I hereby release and agree to hold harmless Joe Photographer, his legal representatives, assigns, and all persons acting under his permission or authority, from any liability by virtue of blurring, distortion, alteration, optical illusion, or use in composite form whether intentional or otherwise, that may occur or be produced in the taking of the pictures, or in any processing tending toward the completion of the finished product, unless it can be clearly shown that the publication thereof were maliciously caused, and produced, and published solely for the purpose of subjecting me to conspicuous ridicule, scandal, reproach, scorn, and indignity.
I hereby affirm by my signature below that all poses, positions and situations enacted including those which involve restraint, fictional depiction of captivity, bondage and other such situations in the still, single, multiple or moving photographic images or pictures, video recordings, audio recordings covered in this release were entered into without force, coercion, or threat whatsoever, and were posed freely by me with my full consent in return for monetary compensation and for my own amusement. I further agree to hold blameless and free of all accusation of such force or coercion Joe Photographer, his legal representatives, assigns, and those acting under his permission and on his authority.
I do hereby warrant that I am of full legal age and have every right to contract in my own name in the above regard. Further, I have read the above authorisation and release, prior to its execution, and I am fully familiar with the contents thereof.

Completion of this form constitutes the release of all rights to photographic
images taken of the model. This release is to discharge any and all claims
and demands arising out of or in connection with the use of photographs /
videos, including any and all claims for libel or invasion of privacy. I hereby
grant the photographer the ownership and full use of any photographs / video
images which are taken.
I am of full age and have the right to contract in my own name.
I have read this release and fully understand the contents. This release shall
be binding upon me and legal representatives.

In consideration of my engagement as a model, upon the terms herewith stated, I hereby give to Photographers name, Photographic Company, his heirs, legal representatives and assigns, those for whom Photographers name, Photographic Company is acting, and those acting with his/her authority and permission:
a) the unrestricted right and permission to copyright and use, re-use, publish, and republish photographic portraits or pictures of me or in which I may be included intact or in part, composite or distorted in character or form, without restriction as to changes or transformations in conjunction with my own or a fictitious name, or reproduction hereof in colour or otherwise, made through any and all media now or hereafter known for illustration, art, promotion, advertising, trade, or any other purpose whatsoever.
b) I also permit the use of any printed material in connection therewith.
c) I hereby relinquish any right that I may have to examine or approve the completed product or products or the advertising copy or printed matter that may be used in conjunction therewith or the use to which it may be applied.
d) I hereby release, discharge and agree to save harmless [photographer], his/her heirs, legal representatives or assigns, and all persons functioning under his/her permission or authority, or those for whom he/she is functioning, from any liability by virtue of any blurring, distortion, alteration, optical illusion, or use in composite form whether intentional or otherwise, that may occur or be produced in the taking of said picture or in any subsequent processing thereof, as well as any publication thereof, including without limitation any claims for libel or invasion of privacy.
e) I hereby affirm that I am over the age of majority and have the right to contract in my own name. I have read the above authorisation, release and agreement, prior to its execution; I fully understand the contents thereof. This agreement shall be binding upon me and my heirs, legal representatives and assigns.
